Summer Camp Returning Dancer Form

Interested in our Adaptive Movement Summer Camps for dancers ages 12-30+? Fill out the form below so we can get to know your dancer and learn how to best support them during our camps. This form is for dancers who have danced with the AIM Initiative before. If you have never danced with the AIM Initiative and would like to join us for a summer camp, click here. Please note: all responses are confidential and used for classroom support purposes only.

Our Summer Adaptive Dance Camps will include time for adaptive dance, music therapy, adaptive theater, and snack/social time. Camps will be from 10 AM-1 PM or 12:30-3:30 PM Monday through Thursday for the week designated. For more information on our summer camps, click here.

Dancers will be required to bring their own snacks. Can your dancer feed independently? Please share what staff needs to know to provide the best support. Please include if your child needs G-Tube support.

Will you be sending emergency medication with your child? Please describe the emergency protocol to follow. As a reminder, we do not currently provide medical care or administer medication.
